Guidance for the Management of symptoms in adults with Heart Failure at the end of life

This regional guidance has been developed for healthcare professionals managing heart failure symptoms at end of life.
It provides specialist interventions for heart failure symptoms which enable patient management at end of life, early care planning, device deactivation and encourages onward referral to those palliative supportive services available within the community setting.
It has been co-produced by a range of clinicians from across all five NI Health and Social Care Trusts, including from cardiology, palliative care, general practice and community nursing and endorsed by Palliative Care Programme Board, Northern Ireland.
